• Veteran must be permanently disabled due to a service-related condition, missing in action, or deceased from service-related causes

First-Time Users:
Apply online at  and complete VA Form 22-5490 (Application for Survivors鈥� and Dependents鈥� Educational Assistance) to receive your Certificate or Letter of Eligibility.

Returning Users:
If you have previously used VA education benefits, complete VA Form 22-1995 (Change of Program or Place of Training) at  to receive an updated Certificate of Eligibility.

• Submit the following items to the 91吃瓜网 Financial Aid Office:

  • Certificate of Eligibility
  • Official transcripts from all previously attended colleges or universities
  • VA Request for Certification Form
  • VA Degree Plan

  Degree plans are completed by your 91吃瓜网 advisor.
